This Privacy Policy is prepared by Rare Form Movement and whose registered address is 46 Cross Parkway, Burlington, VT 05408. (“We”) are committed to protecting and preserving the privacy of our visitors when visiting our site or communicating electronically with us.

This policy sets out how we process any personal data we collect from you or that you provide to us through our website and social media sites. We confirm that we will keep your information secure and that we will comply fully with all applicable United States Data Protection legislation and regulations. Please read the following carefully to understand what happens to personal data that you choose to provide to us, or that we collect from you when you visit our sites. By submitting information you are accepting and consenting to the practices described in this policy.

We may collect, store and use the following kinds of personal information about individuals who visit and use our website and social media sites:

This includes information you provide when you submit a contact/enquiry form. The information you give us may include, but not limited to, your name, address, e-mail address and phone number.

We will use all reasonable efforts to ensure that your personal data is not disclosed to regional/national institutions and authorities, unless required by law or other regulations.

Unfortunately, the transmission of information via the internet is not completely secure. Although we will do our best to protect your personal data, we cannot guarantee the security of your data transmitted to our site; any transmission is at your own risk. Once we have received your information, we will use strict procedures and security features to try to prevent unauthorised access.

Your rights – access to your personal data

You have the right to ensure that your personal data is being processed lawfully (“Subject Access Right”). Your subject access right can be exercised in accordance with data protection laws and regulations. Any subject access request must be made in writing to rareformmovement@gmail.com. We will provide your personal data to you within the statutory time frames. To enable us to trace any of your personal data that we may be holding, we may need to request further information from you. If you have a complaint about how we have used your information, you have the right to complain to the Information Commissioner’s Office (ICO).

Please find the complete PAR-Q and PAR-Q+ survey HERE. If you answer "Yes" to any of the following questions, follow the link to take the more comprehensive survey. Working with Rare Form Movement should only be done by healthy individuals who have successfully cleared this screening.


  1. 1. Has your doctor ever said that you have a heart condition and that you should only do physical activity recommended by a doctor?
  2. 2. Do you feel pain in your chest when you do physical activity?
  3. 3. In the past month, have you had chest pain when you were not doing physical activity?
  4. 4. Do you lose your balance because of dizziness or do you ever lose consciousness?
  5. 5. Do you have a bone or joint problem that could be worsened by a change in your physical activity?
  6. 6. Is your doctor currently prescribing drugs (for example, water pills) for your blood pressure or heart condition?
  7. 7. Do you know of any other reason why you should not do physical activity?

If You Answered Yes

If you answer yes to one or more PAR-Q questions, the next step is to take the PAR-Q+, which is a series of 10 follow-up questions. These more in-depth questions ask about whether you have specific health conditions, such as arthritis and cancer.

Answer yes to any of these and it is recommended that you consult a physician before taking a fitness test or substantially increasing your physical activity. Ask for medical clearance along with information about specific exercise limitations you may have. In most cases, you will still be able to do any activity you want as long as you adhere to some guidelines.

When to Delay an Exercise Program

  • If you are not feeling well because of a temporary illness, such as a cold or a fever, wait until you feel better to begin exercising.
  • If you are or may be pregnant, talk with your doctor before you start becoming more active.

If You Answered No

If you answered no to all the PAR-Q questions, you can be reasonably sure that you can exercise safely and have a low risk of having any medical complications from exercise. It is still important to start slowly and increase gradually. It may also be helpful to have a fitness assessment with a personal trainer or coach in order to determine where to begin.

Keep in mind that if your health changes so that you answer "Yes" to any of the PAR-Q questions, tell your fitness or health professional. You may need to change your physical activity plan.




Rare Form Movement practices are designed for healthy students without active injury concerns. This work is not intended to address personal injuries or replace practices like Physical therapy. Please check with your doctor to make sure you are able to safely practice physical exercise. You are responsible for keeping yourself safe in the practice. Don't push your body or over exert yourself.
